Service Provider Position

POSITION SUMMARY

Home Care (HOHHC), you will play a vital role in facilitating client/’consumer’s’ daily living activities and supporting their independence and well-being. Your responsibilities will encompass a range of tasks aimed at assisting consumers with various needs, promoting their dignity, and ensuring their safety and comfort.

JOB RESPONSIBILITIES AND KEY FUNCTIONS

  • Facilitate consumers in daily living activities, including personal hygiene, dining skills, housekeeping skills, social skills, and community access.
  • Provide direct support and assistance in training activities as outlined in HOHHC’s activity schedules.
  • Aid in mobility for individuals using wheelchairs and other assistive devices.
  • Assist with bedroom maintenance, teach housekeeping skills, and educate on clothing care.
  • Support meal service, teach dining skills, and promote social skills and personal dignity during meals.
  • Administer medications and treatments following company policy.
  • Act as a Primary Person for assigned clients/customers, ensuring their well-being at all times.
  • Attend and complete in-service training sessions as scheduled.
  • Serve as a role model and mentor to fellow service providers/caregivers.
  • Complete data collection, observation notes, and program reports as per agency policy.
  • Assist consumers in transportation as needed.
  • Participate in the Individual Support Team to provide input toward the consumer’s Individual Support Plan (ISP).
  • Monitor consumers’ activities to ensure safety and health care practices are observed, including universal precaution practices.
  • Contribute to the daily operation of the home in areas such as grocery shopping, overseeing consumers’ money, and monitoring house medications.
  • Treat consumers with dignity and respect, promoting their independence, choice, and empowerment.
  • Perform other ad hoc duties as assigned.
  • Possess a responsible, detail-oriented, results-driven work ethic.

EDUCATION AND QUALIFICATIONS

  • High School Diploma or GED is mandatory.
  • Must be eighteen (18) years of age.
  • Valid driver’s license and a good driving record required.
  • Bi-lingual (English/Spanish/French) preferred but not necessary.
  • At least one year’s experience in caregiving and/or experience in supporting/assisting those with disabilities in any setting preferred.
  • Effective communication skills, both oral and written.
  • Satisfactory completion of required background checks and screenings.

  • General residential conditions.
  • Driving a car daily, sometimes distances of 30 miles one-way to work site.
  • The job may require standing, walking, bending, and lifting heavy objects from 10lbs – 50lbs.
  • Work may be conducted indoors or outdoors, exposed to various weather conditions.
  • Adherence to safety protocols and use of protective equipment as required.

WORK SCHEDULE

Monday – Friday work shift. Hours vary from four (4) to eight 8 hours (8) work shifts and can require overtime as requested. This may include weekend hours pending client/customer’s requests.

PAY RATE

$10.00 – $12.00 per hour or $20,800.00 – $24,960.00 annually.
